肺鳞30月，父亲永远地走了

! t( w* Z: p1 s5月22日：今天上午咳出好多血，看得我害怕，吃了3颗云南白药好点了。
7 v0 O) }7 z$ u) Z/ C3 k! }' x9 M验血报告出来了，在电脑上看的，胆红素、血小板 、凝血都是正常的。那咳血是什么原因呢？今天才知道老爸从5月2日起一直在吃云南白药，最近停了6天。
血常规忘了看了，但医生有说过是正常的。
: Q5 U+ t: x) Y) o- s今天做了增强CT，报告要明天出来，今夜注定要辗转反侧了。。。

在CT报告出来前，无法区分是肿瘤进展了还是特罗凯的副作用，不管是哪种情况，都要停药

每隔一阵子就会出现一个处理很棘手的状况

5 [; G) i( V. l, r3 I后续打算：
1) CT示好转或稳定，则处理好咳血后继续用特，止疼药改成曲马多，用川贝枇杷膏处理肺热咳痰；
* @/ T& C- H* r% a$ z. l- C( ?2) CT示进展，则用吉西他滨或多西他赛化疗2周期，然后改用2992；

上次最后一次化疗结束是去年11月16日，方案是紫杉醇+卡铂，3个月后进展；
& x) f. P! Q$ L7 Q8 t2 Z+ o1 F考虑已经用过紫杉醇，这次如果要化疗，准备先用吉西他滨，敢不敢加顺铂？ 还是就吉西他滨单药？目前体力尚可。

" q- i7 I  H: V4 [+ l5月22日的CT报告如图，肺部肿瘤稍微增大、后肋骨转加重（胸骨骨质破坏稍好转）、产生胸水和心包积液；

分析和教训：
) {. g9 A* A) }1) 5月2日开始咳血，咳嗽增加，其实已经是有耐药的迹象了，但老爸人在老家，从未说过一直在吃云南白药；用镇咳药和云南白药盖住了症状，但盖不住肿瘤本身；
2）云南白药“可明显促进血管内皮生长因子(VEGF)的生成”，和抗肿瘤的方向背道而驰，临时止血可以，长期吃肯定不好；郁闷的是，在开始治疗前，还不能停。
3) 特不能说完全耐药，毕竟肿瘤增大幅度很小，但不能再等了，要化疗打压；没准以后还有机会上特；

周二开始天天和医院联系，要到周五下午才有床位，下周一开始治疗。我想还是下周一再去吧。

请教大家后面的方案次序：
化疗还可以用泰索帝（多西他赛）、泽菲（吉西他滨）
靶向还可以用2992、凡德他尼
目前得先用化疗打压一下，然后该用什么次序呢？还有希望回到特罗凯吗？

# g- C/ w" I9 F. S, M184不敢用了，上次用了骨痛加重，副作用难以耐受（喝水都吐）、极度乏力；

另外，第五后肋骨肿块变大，靠近脊髓，风险很大。2月份已经做过姑息放疗，特有效期间肿块退缩，现在不知道该怎么办了。
唑来膦酸每月一针，胸骨骨质破坏稍有好转，后肋骨却没有。

有关凡德他尼，
3 Q/ h7 _7 A% p$ G) _, {1） 有效率不比厄洛替尼高，但副作用更明显。

也有资料显示凡德他尼不能延长总生存期。

当然现在更关心特耐药后，凡德会不会有效。

已用过EGFR-TKI治疗的，凡德不能获益:

KRAS突变，多吉美才比较靠谱?

补充几个结论：
1)  临床试验亚组分析中，亚洲人群未能从西妥昔单抗联合化疗中获益。《肺癌化疗与靶向治疗》，廖美琳。
2) BATTLE的报告中，凡德对KRAS突变的有效率为0。
3) BATTLE报告中索拉菲尼对多种突变（EGFR、KRAS、VEGF、RXD/CycD1）和无以上突变的都有一定的有效率，尤其对KRAS突变有效率高达79%。
) S5 f2 h6 u# G6 Q, ^% U# K4) KRAS突变易腺癌为主，鳞的比较少，如果要试药，腺应该优先试用索拉菲尼（多吉美）。
5) 凡德总体有效率和厄洛替尼相当，但对KRAS突变、无其他突变的有效率为0，但厄洛替尼对KRAS突变、无突变都有一定的有效率。

Results: Gefitinib induced down-regulation of thymidylate synthase and E2F-1 in gefitinib-resistant NSCLC cells with MET amplification but not in those harboring the T790M mutation of EGFR. The combination of 5-fluorouracil and gefitinib synergistically inhibited the proliferation of cells with MET amplification, but not that of those with the T790M mutation of EGFR, in vitro. Similarly, the combination of S-1 and gefitinib synergistically inhibited the growth only of NSCLC xenografts with MET amplification.

Conclusions: Our results suggest that the addition of S-1 to EGFR-TKIs is a promising strategy to overcome EGFR-TKI resistance in NSCLC with MET amplification.

事实上我们无法知道到底什么原因引起的EGFR-TKi耐药。此外鳞一般是TS高表达，如果变成了低表达，用S-1还不如用培美。
